Patient data from the Vale Of Neath Practice (including data extracted from our GP system) will be repatriated from NHS England and analysed by Digital Health and Care Wales (DHCW) to provide aggregated, anonymised outputs that will support clinicians, networks and Local Health Boards to improve the quality of diabetes care provided.
